Fifth Avenue Counseling & Consulting provides SAP initial and follow-up alcohol and drug evaluations for persons who are employed in safety-sensitive positions as defined by the Department of Transportation. We provide this service to individuals and businesses operating under the Federal Motor Carrier Safety Administration, the Federal Transit Administration, the Federal Aviation Administration, the Federal Railroad Administration, the United States Coast Guard and the Pipeline and Hazardous Materials Safety Administration. Fifth Avenue is committed to following the DOT rules for the initial assessment and follow-up assessment with the goal to make the nation’s transportation system safer for the American public.

Our goal is to complete the initial assessment and to make the mandatory treatment referral within one week. We pledge to offer the initial assessment within 48 business hours of the first telephone contact with the person referred for the SAP service. We will, when possible, next try to match the safety-sensitive position employee to treatment covered by their insurance plan. We will select treatment programs that offer the best intervention for the substance abuse problem or failed random urinalysis. Our goal with our clients is to change the substance use behavior of employees working in safety-sensitive positions. Our rates are competitive and our service strives to be exceptional.
